The note states that the Ms. was commissioned and donated by baḥr nagāśi Tawalda Madḫǝn during the time of King Yoḥannǝs IV (r. 1872-89) and Metropolitan Ṗeṭros IV (in tenure 1881-1917). The hand of the note is probably the same as that of the donation notes in UM-013 and UM-029.

Condition
The leather cover is torn off from the spine, partly from both boards. The bottom margin of check the viewer48 has been enlarged by sewing a piece of parchment onto it.
Binding
Two wooden boards covered with reddish-brown tooled leather (partly missing). Two pairs of sewing stations.
Binding decoration
Traces of endband stitches are visible on the spine fold of the quires (small holes with remains of threads, close to the head and the tail of the codex).
